The care of information of patients:
Information about you may be paper-based or housed on computers at this practice of Dr RL Melvill, 212 Constantiaberg Mediclinic, Burnham Rd., Plumstead.
Appropriate measures are taken to prevent disclosure of information to unauthorised persons.
The following personal information about patients is recorded:
Standard demographic information is recorded once this is provided by the patient or responsible person. Information relating to the patient’s medical history, laboratory investigations, and radiological studies as well information from other medical service providers may be retained. This information is required in order to ensure effective neurosurgical care of the patient.
This practice shall disclosure your personal information, with your permission, to the following entities:
- Other medical service providers who are or will be involved with your care.
- My administrative staff for the purpose of submitting medical reports, keeping records and submitting invoices for services rendered.
- Your medical aid or insurer for the purpose of submitting an invoice for services rendered.
- Your insurance companies for the purpose of making claims from that insurance company.
- Your medical information may be disclosed during the course of academic meetings, however this information is always anonymised.
Your personal and medical information shall be disclosed to your attorneys in the event of Dr Melvill being requested to compile medicolegal reports.
